Membership Forms:
Membership Agreement All class participating members are required to submit this form annually.
Associate Membership Agreement Families who would like to be included in iFamily events but not attend weekly classes and who would like to receive notifications of iFamily happenings through the iFamily Yahoo group, may submit this form.
Standards of Moral Conduct All members, class participating and associate, are required to submit this form annually.
Parent Off-Site Agreement All adult members with children attending classes are required to submit this form annually.
Mentor Forms:
Mom School Proposal Here is the form used for submitting to the iFamily Board a class you would like to teach.
Mentor Agreement Prospective mentors are required to submit this form at the time of each class proposal.
Process To Become A Class Mentor Instructions
Youth Mentor Responsible Agreement All youth mentors are required to name an adult who will be responsible to oversee the youth’s class according to the board’s specifications.
